Ages and Stages Questionnaires, Third Edition (ASQ-3), is a series of screening questionnaires for children ages 1 month to 5-1/2 years old. We use the ASQ-3 to make sure your child is developing well and track your child’s progress in important developmental areas such as speech, physical ability, social skills, and problem-solving. The ASQ-3 can help us identify your child’s strengths, as well as any areas where they may need extra support.
Complete Before Each Well-Child Visit
For your convenience, the ASQ-3 is available online. Complete the ASQ-3 questionnaire that is appropriate to your child’s age at home, in advance of your well-child visit. Doing so will enable your child’s healthcare provider to review the information before you arrive, and spend more appointment time focused on you and your child.
