Sign up for a MyChart account
- Online: If you have been a patient at Glencoe Regional Health, Allina Health, or an affiliated health care system, sign up online.
- In person: Ask registration staff or a nurse to activate your MyChart account. They will send you a website link with an activation code to the email address or cell phone number you provide. If you received a MyChart activation code, enter it here.
- By mail or fax: Print the MyChart Sign-Up Form (PDF), complete it, and return it by mail or fax.

Proxy Access – Manage a Loved One’s Care
With permission, a user can view and manage health information of a minor or an adult within their care.
Child Proxy
Allows a parent or guardian access to a MyChart account of a child or minor under the age of 18. To gain access, print, complete, and return the Child Proxy Form (fillable PDF). Proxy access can also be requested through the MyChart account.
Adult Proxy
Allows a caregiver or family member access to an adult’s MyChart account. To sign up for yourself and an adult whose medical care you manage, print, complete, and return the Adult Proxy Form (fillable PDF). The patient (or authorized person) must sign the proxy form to authorize release of his or her medical information. Proxy access can also be requested through the MyChart account.
MyChart Account Features
MyChart provides a secure way to manage your healthcare online. You can:
- Send a non-urgent medical question
- Schedule, cancel, and receive reminders for clinic appointments
- Receive results for most lab and imaging tests
- Get after visit summaries and discharge instructions for clinic appointments and select hospital services
- View and print immunization history
- Request medical records
- Receive reminders to schedule recommended preventive screening tests
Schedule Appointments in MyChart
Select appointment types can be scheduled online via MyChart with your established providers:
- Non-urgent office visits
- Yearly adult physical exams
- Well-child exams
- Routine screening mammograms
For all other appointments, please call 320-864-7816 for scheduling assistance. We want to make sure you have enough time during your appointment for you to address your health concerns with your provider.
MyChart Scheduling Guidance
OFFICE VISIT (general illnesses and medication refills)
In MyChart, select Primary Care (Family and Internal Medicine), then select Office Visit
Call 320-864-7816 to schedule if:
- you need to see a provider immediately
- you have multiple health concerns to address
- you want to discuss mental health medications for depression, anxiety, etc.
- you want to schedule a lab-only or a nurse-only appointment (for example: vaccinations)
ADULT PHYSICAL (ages 18-64)
In MyChart, select Primary Care (Family and Internal Medicine), then select Adult Physical
Call 320-864-7816 to schedule if:
- you want to schedule an adult physical within 365 days of your last one
WELL-CHILD EXAM (ages birth-21)
In MyChart, select Primary Care (Family and Internal Medicine) or Specialty > Pediatrics, then select Well-Child Exam
MAMMOGRAM
In MyChart, select Mammogram
Call 320-864-7816 to schedule if:
- you are experiencing breast symptoms
- you want to schedule a mammogram within 365 days of your last one
